work.---The Community Health Ambassador--is a pivotal member of the
Landmark Interdisciplinary Team. This role actively engages with
patients both in the home and by telephone. The incumbent will use
compassion and empathy to earn patients' and caregivers' trust,
persistence to support and advocate for them, and excellent
communication skills to build impactful relationships.This position
is full-time, Monday - Friday. Employees are required to work our
normal business hours of 8:00am - 5:00pm. It may be necessary,
given the business need, to work occasional overtime and/or
weekends.--We offer 4-6 weeks of training.Primary
Responsibilities:
- Conduct home visits and/or phone or video calls to patients to
collect clinical and psychosocial information, and carry out
designated tasks, including but not limited to:
- Resource referral and connectionQuality measure documentation,
support, and follow upTelemedicine facilitation and set upCollect
information regarding patient conditions using standardized
checklists and templates.Partner with Landmark interdisciplinary
team (providers, nurses, social workers, etc.) to drive improvement
in patient outcomes and satisfactionActively participate in weekly
interdisciplinary team meetingsReports directly to the Supervisor,
Clinical Nurse You'll be rewarded and recognized for your

provide development for other roles you may be interested
in.Required Qualifications:
- High School Diploma / GED (or higher) OR equivalent work
experience
- 3+ years of experience in any of the following job categories
or categories similar to: Social Worker; Home Health Aide;
Certified Nursing Assistant; Certified Medical Assistant;
Medication Aide; Health Coach; Health Navigator; Healthcare
Technician
- Experience with Microsoft Office products (Word, Outlook, and
Excel)
- Access to reliable transportation required; if you are driving
a vehicle, you must comply with all the terms of the Landmark Motor
Vehicle Safety policy
- Ability to conduct independent home visits
- Ability to work our normal business hours of--8:00am to
5:00pm,--Monday - Friday
- Full COVID-19 vaccination is an essential job function of this

- Geriatric or primary care experience, or experience in
institutional settings of care such as long-term care/skilled
