Job Description Job Description About the Role: The Case
Management Registered Nurse (RN) plays a critical role in
coordinating and managing patient care to ensure optimal health
outcomes. This position involves assessing patient needs,
developing individualized care plans, and collaborating with
multidisciplinary teams to facilitate effective treatment and
resource utilization. The RN will serve as a liaison between
patients, families, healthcare providers, and community resources
to promote continuity of care and support patient advocacy. By
monitoring patient progress and adjusting care plans as necessary,
the Case Management RN helps reduce hospital readmissions and
improve overall patient satisfaction. This full-time role requires
a compassionate, detail-oriented professional dedicated to
enhancing the quality and efficiency of healthcare delivery across
diverse patient populations in the United States. Minimum
Qualifications: Current and valid Registered Nurse (RN) license in
the United States. Bachelor of Science in Nursing (BSN) or
equivalent nursing degree. Minimum of two years of clinical nursing
experience, preferably in case management or a related healthcare
setting. Strong knowledge of healthcare systems, insurance
processes, and patient care coordination. Excellent communication,
organizational, and critical thinking skills. Preferred
Qualifications: Certification in Case Management (CCM) or
Accredited Case Manager (ACM) credential. Experience working with
diverse patient populations and chronic disease management.
Familiarity with electronic health records (EHR) and case
management software. Advanced training in patient advocacy,
motivational interviewing, or health coaching. Master’s degree in
Nursing, Healthcare Administration, or a related field.
Responsibilities: Conduct comprehensive assessments of patients’
medical, psychosocial, and environmental needs to develop
personalized care plans. Coordinate with physicians, specialists,
social workers, and other healthcare professionals to implement and
monitor care plans effectively. Facilitate communication among
patients, families, and healthcare teams to ensure understanding
and adherence to treatment protocols. Identify and connect patients
with appropriate community resources, support services, and
follow-up care to promote recovery and wellness. Monitor patient
outcomes and adjust care strategies as needed to optimize health
results and prevent unnecessary hospitalizations. Maintain accurate
and timely documentation of case management activities in
compliance with regulatory standards and organizational policies.
Educate patients and families about disease management, medication
adherence, and health maintenance to empower informed
decision-making. Skills: The Case Management RN utilizes clinical
expertise and critical thinking skills daily to assess patient
conditions and develop effective care plans tailored to individual
needs. Strong communication skills are essential for collaborating
with multidisciplinary teams and educating patients and families
about treatment options and health management. Organizational
skills enable the RN to manage multiple cases simultaneously while
ensuring compliance with healthcare regulations and documentation
standards. Proficiency with electronic health records and case
management software supports efficient tracking of patient progress
and resource allocation. Additionally, problem-solving abilities
help the RN address barriers to care and coordinate timely
interventions that improve patient outcomes.
