Job Description We are seeking experienced Pharmacy Specialists
to support either Emergency Medicine or Oncology service lines.
These roles provide advanced clinical pharmacy expertise, direct
patient care collaboration, and interdisciplinary partnership in
high?acuity and specialty care environments. Ideal candidates bring
strong clinical judgment, prior specialty experience, and the
ability to integrate seamlessly into multidisciplinary teams.
General Summary The Pharmacy Specialist delivers comprehensive
clinical pharmacy services for patients in the Emergency Department
and Oncology settings. This includes medication optimization,
treatment plan review, therapeutic monitoring, patient education,
interdisciplinary rounding, emergency response participation, and
quality/safety initiatives. The specialist also serves as an
educator and preceptor for residents, students, and clinical
trainees. Essential Responsibilities Clinical Care & Consultation
Conduct comprehensive medication reviews including dosing, organ
function assessment, interactions, IV?to?PO transitions,
therapeutic monitoring, and safety considerations. Participate in
Emergency Department responses including trauma, stroke,
psychiatric crises, toxicology events, medical resuscitations, and
high?risk procedures. Review and optimize oncology care plans in
the EHR to ensure accuracy, guideline adherence, and appropriate
supportive care. Assist with antimicrobial stewardship including
culture review and patient call-backs for untreated or
inappropriate empiric therapies. Identify and resolve
medication-related problems using clinical presentation, lab data,
and patient history. Patient Education Provide patient-specific
counseling using the teach-back method for discharge medications,
oral oncolytics, and chemotherapy as needed. Support transitions of
care for high?risk or complex patient populations.
Interdisciplinary Collaboration Serve as a subject matter expert in
Emergency Medicine and Oncology pharmacotherapy. Participate in ED
and Oncology rounds, contributing evidence-based recommendations to
optimize treatment. Support provider teams with medication
information, guideline interpretation, and pharmacotherapeutic
expertise. Quality, Safety & Optimization Review medication error
reports, identify trends, and recommend workflow/process
improvements. Promote rational, cost-effective medication use with
a focus on cost avoidance and optimization across both service
lines. Participate in departmental and service line meetings to
drive clinical excellence and safety initiatives. Education &
Leadership Provide education to pharmacists, nurses, medical
residents, Oncology Fellows, and other healthcare professionals.
Precept pharmacy residents and students. Maintain awareness of
community and professional resources through engagement with state
and national organizations. Qualifications Prior experience in
Emergency Medicine or Oncology pharmacy (role specific). Board
certification (BCPS, BCOP, or relevant specialty) strongly
preferred. Must hold a Delaware Pharmacist license. Strong
collaboration skills, clinical reasoning, and ability to perform in
high-acuity clinical settings.
