is to deliver exceptional, personalized care that enhances health,
reduces costs, and empowers individuals to lead fulfilling lives.
With a strong foundation in clinical assessment and care

traditional barriers by meeting members in their own homes. We
strive to improve the quality of care for older adults and other

identifying chronic conditions that might otherwise remain

one hour, our providers assess a member’s current health status,
medical history, medication adherence, social environment, and

into a member’s overall health that are often missed during
standard office visits. The Matrix Comprehensive Health Assessment
(CHA) enhances care quality and allows us to address multiple care

case managers, social workers, family members, key caregivers, and
other medical personnel as needed. Work alongside Primary Care
Physicians (PCPs) for patient education and follow-up as required.
